How to sign up for trips

Here is the prison tour schedule.

Here is how to sign up:

I will announce the signup period about 40 days prior to each tour. Then, you 
must send me email with the following information EXACTLY as formatted below:

--Last name, first name, middle initial (if you don't have one, put NMI)
--Social Security Number
--Date of birth
--Race
--Sex
--Home (permanent) address
--Whether you've lived in a foreign country (and when)
--Any other names you've used (maiden name, aliases, etc)

EXAMPLE:

Jones, John R.
Soc: 999-13-9999
DoB: October 18, 1981
Race: Hispanic (or mixed-race, or White, or whatever)
Sex: M
Permanent Address:
  222 N. Second street
  Springfield, IL 40000
Foreign Residence: None  
  (or if you have:  Lituania, 1999-2001
Aliases: None
  (or:  pre-married name: Susan Susanski

This information is required, so be sure that it is complete and accurate.
